Working together with families, Liberty POST Ongoing Service Coordinators (OSCs) use their knowledge and access to community resources and service delivery systems to advocate for children diagnosed with language and motor delays, autism, cerebral palsy, Down syndrome, muscular dystrophy, and deafness.

 

Service Coordinators partner with parents in identifying and implementing family goals and assist families in obtaining evaluations and therapeutic services that support and nurture a child’s development. Services are provided in the child’s natural environment which may be at home, child care, Early Head Start, or other community programs if appropriate.

Role of Service Coordinator

  • Help parents develop a family-centered IFSP
  • Provide information and advocacy
  • Facilitate evaluations, link families with resources
  • Coordinate across agency lines
  • Monitor due process
  • Monitor delivery of services (such as speech, physical, and occupational therapies, special instructors, nutritionists, and social workers)
  • Assist with other related issues, such as obtaining information on child development, enrolling in health insurance, and other family concerns.
